Growth for telehealth brands, ad click to third refill

Scale the telehealth businessyou already have.

Firstfill is the growth partner for telehealth brands. We work the whole path from ad click to third refill, so the budget you already spend brings more patients and each one is worth more.

yourbrand.com

The first thing a patient sees after the ad. One brand config renders every page, so a price or offer change lands everywhere at once.

[ 04 ] What we move

Two numbers. How many patients, and what each is worth. Everything we ship moves one of them.

At this volume one point of conversion is seven figures, so nothing on a page is cosmetic. Some weeks the growth is in one field on the intake. Some weeks it is a new upsell or a portal that patients buy from every month. We work both every week, and live traffic decides which version stays.

Why funnels stall

Day 30. Still the default funnel. The package got you live. It didn't get you tuned.

A business-in-a-box gets a clinic launched in weeks: LegitScript, a provider network, a pharmacy, a CRM, and a funnel that comes preconfigured. That funnel is the same on every account. A month in, it is still what your patients are hitting, and every row below is a place the money you spend on traffic is quietly leaving.

  1. 01Landing page
    Shipped at launch

One page from the template library. Platform copy, a stock hero photo, a Start button.

    Day 30

Every ad, audience and keyword lands on it. The person who searched a drug price and the person who tapped a testimonial reel read the same headline.

    On the table

Half the clicks you pay for land on a page written for the other half. The price-shopper leaves without a price, the reel-tapper without a story, and the click is spent either way.

  2. 02Intake
    Shipped at launch

The stock questionnaire: every question the template ever needed for any product, in the order it came.

    Day 30

On a phone, at night, a patient is asked about products you don't sell before being asked what they're trying to lose. Lock the screen and it starts over.

    On the table

Every question is a place to stop, and every stop is a paid click walking out. Nobody trimmed the ones that don't gate eligibility, so a dropout on question 31 costs exactly what one on question 3 does.

  3. 03Checkout
    Shipped at launch

One plan, one price, one button. Card is charged first; the provider reviews after.

    Day 30

The patient pays before anyone has said they qualify. When they don't, a refund follows, then a support ticket, then a review.

    On the table

Charging ahead of the provider turns every decline into a refund, a ticket and a review. You pay the processor on the way in and on the way out, and the patient tells people.

  4. 04Upsells
    Shipped at launch

None. Checkout ends on a thank-you page and a login link.

    Day 30

Someone who just committed to a three-month plan sees a receipt. The add-on, the nausea kit, the longer term — not offered.

    On the table

The minute they're most willing to buy is the minute the funnel stops selling. Every add-on that isn't offered is money left with a patient you already paid full price to acquire.

  5. 05After checkout
    Shipped at launch

A login to the platform portal and an email sequence nobody has opened since launch.

    Day 30

Refill reminders fire on the platform's calendar, not the pharmacy's ship date. The patient emails support to ask where the vial is.

    On the table

A patient who leaves in month two was paid for in full and never reached margin. That looks like a retention problem. It's a portal that can't say when the box ships.

  6. 06Split tests

    Split tests running: 0

There is no second version of anything. Every stage is the one it launched as, and the only way to learn whether it is good is to keep paying for traffic into it.

    On the table

Every dollar of traffic into an untested stage buys the same result as yesterday and teaches you nothing. Whatever the better version would have converted, you're paying that difference daily.

From here

Firstfill takes it from there: landing, intake, checkout, upsells and portal rebuilt as one system, each stage running a variation against the one you launched with.

Cost of waiting

The gap is not waiting for you. It bills you daily.

While the losing version stays live, the difference between the two keeps leaving. This is that difference, running in real time.

Patient portal shop

A shop inside the patient portal. Refills are already a habit.

A patient opens the portal every month for dose and refills. Put supplements, hair kits, gut bundles and clinician-reviewed peptides on that same page, shipped in the same box. Try it: add a few, then run the basket across your own patient base.

[ 05 ] How we work with you

The growth team you would hire, already built.

Firstfill is not three account managers hiding behind a name. Senior hands do the work: Stripe integrations, HIPAA-aware infrastructure, patient intake for regulated products, and the weekly testing that moves the numbers after launch.

Where a project needs a specialist we don't keep on staff, a compliance review, a payments edge case, a design pass, we bring in someone who's done exactly that before, scoped to the project, not billed as overhead.

[ 06 ] Work

Two funnels we run, minus the names.

Both clients are under NDA, so the brand and the numbers stay out of it. The build does not.

[ 01 ] Multi-brand telehealth group

Two consumer brands and partner brands, one funnel stack.

  • Around 45 landing pages render off one brand config, so a price or an offer changes once.
  • Intake, checkout and the offers after checkout rebuilt on the client stack.
  • One patient portal serves both brands from a single codebase: dose, refills, clinician thread.
  • Portal and intake sync into the EMR and the lifecycle email tool the care team already uses.
  • Reporting reconciled against settled Stripe charges instead of checkout page views.
  • An abandoned intake gets a signed link back to the exact step, a declined card gets retried.
  • A daily sweep flags stuck orders before a patient emails support.

Running weekly, still shipping.

[ 02 ] GLP-1 weight loss brand

Off a form vendor, onto an intake we can test.

  • Multi-step intake rebuilt in Next.js, with the drop visible per step and per device.
  • Clinical qualification inside the form: disqualifiers, BMI, routing to a live consult.
  • A version that takes the card before the long clinical intake, live against the original.
  • Several funnels at once on one engine: price points, multi-month plans, partner mirrors.
  • Coupons and prices wired to Stripe, so a promo is one change and not six.
  • A second provider integration proven on a test funnel before it saw a real patient.

Shipped, still under test.

The roster

We handleall four.

One contract. One thread. One person who answers.

[ 07 ] Security

Patient data, handled like patient data.

We work under signed BAAs. Health answers never touch analytics or marketing tools, access is scoped per role, and intake only sends what the flow actually needs. If a request would put patient data somewhere it shouldn't go, we flag it while scoping the work, before it's built.

  1. 01Signed BAAs on every telehealth engagement
  2. 02Health answers kept out of analytics and marketing pipes
  3. 03Access scoped per role, not per project

[ 08 ] Pricing

Three ways in. Fixed prices, written down before the work starts.

Your platform keeps doing what it does — provider network, pharmacy, prescriptions. We take the part in front of it: the pages, the intake, the checkout and the tracking, on a stack you own.

  1. Audit$1,500one-timeWe read the live funnel end to end and write down where the traffic drops and what to fix first.
  2. Funnel Takeoverfrom $9,000fixed scopeRebuild landing → intake → checkout → upsells on your own stack, with the numbers reconciled and the first test already running.
  3. Growth Retainer$4,999per monthThe funnel keeps moving: two tests running at any time, something new shipped every month, one thread that answers.
What each one includes →not included, in every option: ad spend, platform fees, provider network and pharmacy.

All 8 questions
Our funnel already converts. What is left?+

The stages nobody has tested yet. Almost every brand doing volume tested the landing page and stopped there, so the intake order, the upsell take, the failed payments, the carts nobody recovers and what happens in month three are still on their first version. At your volume those are worth more than another landing page test, because nobody has taken the first pass at them.

Where do you start, and why there?+

At the one step where a point is worth the most money to you. That might be ad to landing, landing to intake, intake to checkout, checkout to paid, the upsell take rate, or whether people are still refilling in month three. It might not be a page at all: reporting that disagrees with Stripe, failed payments nobody retries, carts nobody recovers, a product or brand that deserves its own funnel. We rank them by what a fix is worth and start with the top one.

How do you move that fast?+

AI does the typing, a senior engineer decides what gets typed and reads every line before it ships. In the time a normal build puts one landing page live and tests it, you get three to five whole themes running against each other, so the same month of ad spend buys several shots at a winner instead of one. Nothing touching patient data ever runs unreviewed.

What happens to our revenue while you rebuild?+

Nothing pauses. The funnel you have keeps taking orders while the new version is built next to it, then traffic splits between the two. Ads keep running, and the version people are already buying from stays up until the new one beats it.

How do you know a winner actually won?+

The count that decides is the charge Stripe settled, not a checkout page view the platform's report calls a conversion. A variation stays up until it has enough orders to separate from the other one. If it never separates, we say so and delete it.

How is it priced?+

Three fixed prices, public before you book anything: a $1,500 audit credited in full against a build, a Funnel Takeover from $9,000 for one stage or the whole funnel, and a $4,999 a month retainer that keeps tests and new pages shipping. Ad spend and platform fees stay yours.
